Spring Camp Equipment Rental
At the LMFA storage facility – 100 Bessemer Road, London.

Requirements:
Impression of credit card – MasterCard or VISA
Driver’s Licence Number
Fee of $45.00 in cash or cheque
 

Equipment Sign Out Dates:
     1. Sunday March 4th, 10 am to 1 pm
     2. Monday March 5th, 6 pm to 8 pm
     3. Thursday March 8th, 6 pm to 8 pm
     4. Tuesday March 20th, 6 pm to 8 pm

Registration Dates
Wednesday January 11 and 25 from 7-9 pm.
Saturday January 14 and 21 from 11 am -2 pm.
Wednesday February 1, 8, 15 and 22 from 7-9 pm.
Saturday February 4, 18 and 25 from 11 am -2 pm.
Wednesday March 7 and 21 from 7-9 pm.
Saturday March 10 and 24 from 11 am -2 pm.

    • Peewee: 2000-2001-2002
    • Bantam: 1998-1999
    • Junior Varsity: 1996-1997
    • Varsity:1993-1994-1995
  • Registration Location:
    Westminster Secondary School – Cafeteria
    230 Baseline Road West, London N6J 1W1
  • Registration Requirements:
    Provide a photocopy of Birth Certificate / Proof of age
    Provide Health Card Number
    Parent / Guardian must complete and sign the registration / waiver form
    if the participant is under 18 years of age
    Camp Fee of $100 – Cash, cheque or money order payable to Forest City Thunderbirds
    Players are responsible for their own equipment for spring camp
