Registration

1. Download and complete the Thunderbird Registration Form.
2. Provide photocopies of your BIRTH CERTIFICATE and HEALTH CARD.
3. Provide a $50 cheque made payable to the Forest City Thunderbirds Football Club.
4. Complete and include all of the above and mail to:

Please note:

• The $50 administration fee is non-refundable. This fee covers insurance and provincial organization fees as well as our pre-season training camp at TD Waterhouse Stadium.
• Once training camp is completed and the team is chosen a player fee of $325 will be collected.
• A parent/guardian signature is required on the completed Acknowledgement of Risk and Release of Liability section before a player is allowed to participate.
